To make an appointment with Team Member Health, please select the appropriate visit category on the drop down menu.
You DO NOT need an appointment for:
work clearances
bloodborne pathogen exposures
communicable disease exposures
new work related injuries
However, you may make an appointment for a work clearance to ensure a shorter wait time.
If you have a new work-related injury that needs immediate evaluation please report to Team Member Health as a walk-in.
PLEASE NOTE: The appointment scheduler ONLY allows you to make appointments NO MORE THAN 30 DAYS IN ADVANCE. If you need an appointment over 30 days away, please check back within the 30-day period, as we open new appointments each week.
