Please provide your information and we will reserve you an estimated time.

NOTE: Reserved visit times are subject to change, due to emergent walk-in needs. For your convenience, please opt in for reminder text messages about your estimated time once you submit your information. You will also receive text messages if there are any unexpected delays.

Visitor Restrictions: One visitor is allowed to accompany a patient (must be 18 or older). 

For any Employer Specialty Services call the office for availability and scheduling.
Please be sure to wear your own face mask when coming to any in-office visit.

RediMed is offering Rapid COVID-19 testing with same day results as available. We currently are not offering testing for an exposure only. If you have been exposed, but don't have symptoms, you can search for a CVS Pharmacy location at:  https://www.cvs.com/minuteclinic/covid-19-testing, Indiana Board of Health location at: https://scheduling.coronavirus.in.gov/Home/LocationSelection

 But first, please make sure you don't need to call 911.
