• The time you chose is not an appointment, but when you'll be in line for your visit.

• Please complete your registration online before coming in. You can upload your ID and insurance info. If you haven't, we'll send a link by text or call us, and we'll assist you.

• You must bring a Valid ID, Insurance Card and a form of payment with you to the clinic

Remember to check your insurance plan for potential costs.

If it's an emergency, call 911 or go to the nearest ER. Our services are not intended for life-threatening issues.

When you arrive, we'll ask you to verify your details.

For rescheduling, give us a call, and we will get you rescheduled.
