The Everett Clinic has implemented safety measures regarding Coronavirus consistent with the CDC and Department of Health recommendations. Please answer the questions as accurately as possible. In some cases, you may be directed to another location during in-person screening due to our enhanced Coronavirus safety measures.

Where to seek care:

If you have any of the following new or worsening symptoms in the last 10 days: Fever, chills, shortness of breath, difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea, vomiting, or diarrhea please visit:

Without Above Symptoms please visit:

Stanwood, Smokey Point, Lake Stevens, Snohomish, Harbour Pointe, Silver Lake, Mill Creek, Bothell, Shoreline
