Credit card on file will be required for all commercial insurance patients prior to being seen. ID and Insurance Card will be requested at every visit. All visit times are estimated and may not be exact.

Please note: Patients with High-Deductible Health Plans (HDHPs) who have not met their deductible may be asked to pay $125 at time of service as an estimated patient responsibility.
