This self scheduling tool, provided by MelroseWakefield Healthcare and Tufts Medical Center Community Care, is open to anyone age 12 and older, who live, work, or study in Massachusetts.
Massachusetts state guidelines

Please note: We are not able to vaccinate family or friends who accompany a patient to a vaccine appointment unless they meet the current eligibility requirements and have a scheduled appointment. For more information please visit mass.gov/covidvaccine.

The vaccine may require one or two doses:
If you require a second dose, your second appointment time will be provided to you when you get your first dose.
Pfizer - 21 days
Moderna - 28 days
Johnson & Johnson - no second dose required
The vaccine you receive will depend on the current supply. You will not be able to choose which vaccine you receive, but we will inform you on the day of your scheduled appointment.

Where to go and what to bring:
On the day of your appointment, the vaccination clinic is at MelroseWakefield Hospital in the Perkins Lecture Hall, right off the main entrance, and please remember to bring your ID, you will need to show it at the door.

Some insurance companies provide reimbursement for administration of the vaccine. You will not personally receive any charge for the vaccine given regardless of your copay or deductible.

I understand that at the vaccination site I may be asked to provide proof of the above. Such proof can include: government issued ID, work badge, employment letter or current pay stub.
